Join us for Pearl Youth Church Camp! July 28-31st. 

Unrivaled Camp 2019 is The Pearl Church's first ever summer camp! Students can expect to have a lot of fun and experience powerful moments in the presence of God. This week is going to be a truly life-changing experience geared for middle school and high school students. 

Your students will meet and be picked up at the Pearl Church, 200 S University Blvd. Denver CO 80209 but camp will be at the YMCA of the Rockies in Estes Park! 

If you are registering multiple students, please fill out this form for your first student and it will then allow you to add additional students on the following pages.

I hereby give permission and acknowledge that my youth will participate in certain activities (“Church activities”) specifically described as UNRIVALED CAMP conducted by, sponsored or attended by THE PEARL CHURCH, its directors, officers, employees, and agents or members (collectively referred to as “Church”). In signing this form, I, (parent or guardian of participant) agree to not hold Church liable for any injury, loss, damage, harm or death that my youth may encounter while (i) participating in the certain Church activities, (ii) transportation to and/or from the Church activities and (iii) any and all activities incident to the Church activities.
I (parent or guardian) am aware and agree to allow my youth to be transported by an approved Church driver to and from the certain Church activities.
In the event that I, (parent or guardian of participant) cannot be reached, my signature below gives my consent and permission for my youth to receive medical attention at the discretion of the Church.
I, (Parent/Guardian of Youth/Participant), by signing below I verify that the above information is correct and give my permission for my youth to participate in all prescribed activities except as noted by me. I also authorize them, in the event of a disciplinary problem, to send my teen home via best available means after contacting us.
